Telehealth Consent Agreement

By booking and attending a telehealth appointment with All Psychiatry, you acknowledge and agree to the terms outlined below.

Private and Safe Environment

Your appointment should take place in a private, safe, and quiet location where your confidentiality can be maintained. If your clinician considers that your environment is unsuitable or unsafe for the consultation, they may end or reschedule the appointment. Our standard cancellation policy may apply.

Identity Verification

To support patient safety and accurate identification, we may ask you to confirm your full name, date of birth, current address, and contact details at the beginning of your consultation.

Nature of Telehealth

The decision to prescribe anymedication remains entirely at the discretion of the treating clinician.

Prescription Requests

You understand that your consultation will be conducted using secure telehealth technology. Your treating clinician will determine whether telehealth is appropriate for your clinical circumstances and may recommend an in-person consultation or referral if clinically required.

Privacy and Confidentiality

All Psychiatry takes reasonable steps to protect your privacy and confidentiality during telehealth consultations. We encourage you to participate from a private location where your conversation cannot be overheard.

Your personal information will be handled in accordance with our Privacy Policy.

Transcription Software

Our clinicians may use secure clinical transcription software to assist with preparing consultation notes and treatment letters. These tools are used solely to improve the accuracy and efficiency of clinical documentation and are subject to appropriate privacy and confidentiality safeguards.

Technical Difficulties

While we make every effort to provide reliable telehealth services, technical interruptions may occasionally occur. If your consultation is interrupted, we will make reasonable efforts to reconnect or arrange an alternative method of consultation, where appropriate.

Prescriptions

The decision to prescribe medication rests solely with your treating clinician following an appropriate clinical assessment. Participation in a telehealth consultation does not guarantee that a prescription will be issued.

Emergencies

Telehealth services are not suitable for medical or psychiatric emergencies.
If you or another person is at immediate risk of harm, please call 000 or attend your nearest Emergency Department immediately.

Consent

By proceeding with your telehealth appointment, you confirm that:

You understand the nature and limitations of telehealth services;

You consent to receiving healthcare via telehealth where clinically appropriate;

You have read and understood this Telehealth Consent Agreement; and

You agree to the collection, use, and disclosure of your personal information in accordance with our Privacy Policy.
